*Exam (D0150), x-ray (D0210/D0330) and healthy mouth cleaning (D1110) are valid for new patients without insurance, Medicare, Medicaid, TRICARE, or a discount dental plan and only for patients with the absence of periodontal disease. If gingivitis or periodontal disease is present, the patient may be offered a similar treatment (D4335 or D4346). Offer may not be combined with other offers.
